Most Valuable Prescription Savings Club Terms and Conditions

 

The Most Valuable Prescription Savings Club is offered to you by Frontier Pharmacy. The following Terms and Conditions govern your participation in the Most Valuable Prescription Savings Club.

 

General Terms of Use

 

This card can be used only for purchases of prescriptions and merchandise at Frontier Pharmacy in West Fargo, ND. This card cannot be redeemed for cash and no change will be given, except as described below. Taxes are the responsibility of cardholder. Certain terms may be void or restricted as required by law.

This card is non-transferable. Any fraudulent or unauthorized use of this card is strictly prohibited by law. Terms and conditions may be changed at the discretion of Frontier  Pharmacy and the program using this card may be terminated at any time.

 

The Most Valuable Prescription (MVP) Savings Club

The MVP Savings Club is a discounted prescription buying program for individuals who have no prescription drug coverage, or insufficient drug coverage. Members are entitled to discounts on cash prices for select medications included in the medication list. Members shall pay a yearly fee of $10 for individuals and $15 for families. Family price includes spouse and dependents under the age of 19. Cards for other adult family members, such as parents or grandparents, will cost an additional $10 per person. Persons receiving benefits from a publicly funded health care program are ineligible for the MVP Savings Club program. By enrolling in the MVP Savings Club you are affirming that you are not a recipient of benefits from a publicly funded program. The Prescription Savings Club is not insurance or an insurance benefit, nor is it intended as a substitute for insurance. It cannot be combined with insurance

 

Prescriptions can be mailed to members of the MVP Savings Club.  A $2.95 flat shipping charge is added to all mailed orders.  Multiple prescriptions can be sent in one order.  Non-prescription medications and general merchandise can be mailed if a prescription is being included in that order.

 

MVP Savings Club members have easy access to ordering refills of their prescriptions.  Refills can be phoned in to our toll-free number, ordered online at www.frontierRx.com, or dropped off at our location.  Members can also sign up for our Auto-Fill program which automatically keeps track of when your prescriptions need to be refilled.  For more details on the Auto-Fill program, visit www.frontierRx.com.

 

The MVP Savings Club Formulary offers select medications in many classes of drugs currently available on the market. Frontier Pharmacy reserves the right to modify the formulary at any time by adding or deleting medications.

Participation in the MVP Savings Club also entitles members to a 10% discount on the purchase of any general merchandise or non-prescription medication from Frontier  Pharmacy.

 

Description of Benefits:

Disclosures: The MVP Savings Club is NOT an insurance plan, a Medicare prescription drug plan, or health insurance policy.

 

As a participant in the program, you are entitled to receive discounts on certain pharmacy products and medical services at Frontier Pharmacy. The MVP Savings Club does not provide you with any discounts or benefits at any other pharmacy. The program benefits are subject to change, modification or substitution without notice. You are solely responsible for payment at the time of purchase of the established price for the prescriptions, or ancillary services you purchase under the program. The range of discounts for medical or ancillary services offered under the program will vary depending on the types of products and services. The program does not make and is prohibited from making payments to Frontier Pharmacy for products or services received by you under the program. You are required and obligated to pay for all discounted prescription drugs, medical and pharmaceutical supplies, services and equipment received under the program, but will receive a discount on certain identified medical, pharmaceutical supplies, prescription drugs, medical equipment and supplies from Frontier Pharmacy.

 

Membership Terms and Cancellation:

 

The Plan is operated by Frontier Pharmacy. You can call 877-364-3932 for more information or visit mvp.frontierRx.com. To join by mail include complete member enrollment form and check payable to Frontier Pharmacy and mail to Frontier Pharmacy, 3306 Sheyenne St, Suite 218, West Fargo, ND  58078. You must pay the membership fees when you sign up for the program in order to enjoy the privileges of the MVP Savings Club. You may cancel your membership at any time. If you wish to cancel your membership, you must do so in writing. If you cancel your membership within 30 days from receipt of your membership card and materials, you will receive a full refund of your membership fees. To cancel, mail cancellation letter and your MVP Savings Club membership card to: Frontier Pharmacy, 3306 Sheyenne St, Suite 218, West Fargo, ND  58078. Your membership will terminate effective as of the date Frontier Pharmacy receives your notice of cancellation. Frontier Pharmacy may cancel your membership and give you a pro rated refund if the MVP Savings Club is discontinued for any reason. If you fail to pay membership fees, as required, or if you violate any of these Terms and Conditions, Frontier  Pharmacy reserves the right to cancel your membership immediately.

 

Any complaints should be directed to Frontier Pharmacy in writing. After investigation of the complaint, Frontier Pharmacy will provide you with the results and a proposed resolution no later than 30 days after receipt of the complaint.

 

Use of Membership:

 

Your membership and benefits are not transferable or assignable without our express written consent. You and Frontier Pharmacy agree that there are no additional third-party beneficiaries under the MVP Savings Club.

 

No Change in Status:

You understand that your prescription medications are dispensed only in accordance with the order of your physician or other health care provider. Membership in this program does not alter or affect your ability to obtain prescription medication or your responsibility to use prescribed medication in accordance with the directions of your physician or other health care provider and pharmacist.

 

Confidentiality:

 

As a pharmacy, Frontier Pharmacy ensures that customer prescription information remains strictly confidential. For information about the privacy of patient health information, please review the Notice of Privacy Practices at http://www.frontierrx.com/Privacy.html.

 

As an extra service to you as a part of your enrollment in this program, we may send emails, direct mailings, or telephone you with offers or information related to your pharmacy care and treatment. Your privacy is very important to us.
